  • By checking here, you are consenting to the use of your electronic signature in lieu of an original signature on paper. You have the right to request that you sign a paper copy instead. By checking here, you are waiving that right. After consent, you may, upon written request to us, obtain a paper copy of an electronic record. No fee will be charged for such copy and no special hardware or software is required to view it. Your agreement to use an electronic signature with us for any documents will continue until such time as you notify us in writing that you no longer wish to use an electronic signature. There is no penalty for withdrawing your consent. You should always make sure that we have a current email address in order to contact you regarding any changes, if necessary.

    ClubCambiati:

    Website Access: More access from our website is included in your membership. Make sure to look for the email giving you access. We recommend that you download the recipe book and schedule your first private consultation to kick off your membership.

    Length of Term: Your ClubCambiati membership shall commence on the purchase date and will auto-renew. If cancelled before 6 months the remaining months will be charged. If cancelled after 6 months, then 30 days’ notice is required in writing. The agreement with auto-renew monthly after the six month initial term.

    ClubCambiati Private: You have access to CambiatiCleanse classes any time, and unlimited Body Composition analyses you also get two Private Consultations with a nutritionist each month that you are not doing a CambiatiCleanse. As well, we are excited to offer a 10% discount on most vitamins! We extend this to as many items as possible, but there a few items that are not discounted. 


    More Boring Stuff AKA General Terms Applicable to All Services:

    Automatic Recurring Payment Failure: If your automatic recurring payment is declined, you will be contacted by phone or email. We may restrict your use of a certain payment method if that method fails multiple times. If your payment fails, you will need to provide another payment method. If you fail to remedy the payments, there may be a penalty for premature termination.

    Changing Automatic Recurring Payment: If you change your Automatic Recurring Payment, including making a change to a new card it will be implemented on the next billing cycle.

    Appointment Changes: We work hard to prepare for your appointment, so please call at least 48 hours prior to your appointment if you need to reschedule. Less than 48 hour notice may result in a loss of a session. Appointments must be completed within 6 months from the sales date unless you have chosen a year-long package.

    Refunds: We want to hold you at the highest performance level and not completing this program would not serve you. No refunds are given because we want the best for you. In addition, we do lots of preparation and pay for lab tests at the beginning of the packages or private sessions. Finally, we book time with nutritionists ahead of time. Don’t worry because you are going to love your results!

    AGREEING TO THESE CONDITIONS: By providing my credit, or debit card or bank account information ("Payment Method"), I AGREE that I have read and understand this Automatic Recurring Payment Agreement.

    In addition, I authorize Cambiati Wellness Programs to:

    Charge you every month for my membership to the specified Payment Method; and I authorize the financial institution for the Payment Method, specified above to charge my account and remit payment for membership to Cambiati Wellness Programs. This authority will remain in effect until I complete the minimum term and give 30 days’ notification in writing, as required under this Agreement, to terminate this authorization. I have read and agree to the terms above including the refund and cancellation terms.
