I, the Applicant, am authorised to make this application on behalf of the Customer. I confirm that the information provided in this form is complete, accurate and correct.

Applicant Declaration
I understand this network connection application is for the provision of an electrical connection to the embedded network and that electricity will not be connected until the installation is completed and tested to all relevant Australian Standards, and the relevant state electricity connection manual.

I certify that I will ensure the electrical contractor is authorised under all relevant acts and legislation to perform electrical work and connect the installation. I state that the electrical installation will be completed in accordance with all relevant acts, standards and legislation and that the electrical installation will be electrically safe and ready for connection by date nominated. I acknowledge that any circuit or equipment able to be energised but operating a switch or circuit protective device will have been tested and is electrically safe, and any outgoing circuits that are connected must be tested.

By signing this form, I the applicant, agree to the following:
1. Receiving all correspondence via electronic means.
2. Guaranteeing that the Customer will pay for all and any fees or costs associated with the connection or application including engineering, approval fees, associated electrical works.
3. Guaranteeing that the Customer will pay for for all and any power used during construction. If this power is not metered due to the applicant not providing timely or accurate information, power will be charged via estimate.
4. Guaranteeing that the Responsible Party will pay for all and any costs associated with the meter installation.
5. The Customer will enter into a supply agreement with the Exempt Network Operator (ENO) for the purchase of electricity from the embedded network.
