Customer hereby authorizes EZEETEL Inc. (“EZEETEL”) to act as an agent on its behalf in the ordering and administration of telecommunication services and facilities. This applies to the addition of new services, rearrangements and removal of services carried out as per the received instructions.
This letter directs EZEETEL to transfer the Telephone Numbers listed below from the current provider and gives EZEETEL full authority to deal with the service provider on the Customer’s behalf with regard to the billing of the Numbers. This direction and authorization will remain in effect until written notice is received by EZEETEL that it has been withdrawn by the Customer.
The Customer acknowledges having transferred the Numbers to EZEETEL and EZEETEL, for all purposes, will control and be responsible to the service provider for payment of all amounts owing in respect to those Numbers. The undersigned acknowledges that the customer had agreed with EZEETEL to cancel such transfer only upon payment in full to EZEETEL of all amounts owed by the Customer for services provided in respect of the Numbers.
The undersigned warrants to service provider that they are an authorized signatory of the Customer and have full authority to authorize EZEETEL to act as an agent on behalf of the Customer.
Please note that you are required to fill the form accurately. Any type of lapse or mistake may result in cancelling the request by EZEETEL. You are required to attach the most recent invoice from the same carrier. The invoice must contain your Full Name, Billing Telephone Number and your Service and Billing Address.
Important things For Local Number Porting.
· The number porting process may take 10 to 15 business days.
· Your current carrier provider decides the transfer date.
· Number transfers only affects incoming calls.
· When your number porting process will be successfully completed, in many cases EZEETEL notify the other service provider to close your account. But it is your sole responsibility to check if your account is closed after porting. EZEETEL does not take the responsibility.
· Please note the EZEETEL’S 911 service is slightly different from the traditional 911. If emergency response center operator is unaware of your location and phone number.
· DO NOT CANCEL SERVICE WITH YOUR CURRENT PROVIDER BEFORE THE NUMBER HAS BEEN TRANSFERRED OR ELSE WE WILL BE UNABLE TO TRANSFER IT SUCCESSFULLY.
· You must cancel the service with your current provide, EZEETEL does not take the responsibility of the cancelling your service or any other charges regarding cancellation or contract penalty.
· EZEETEL cannot port your number in cases including but not limited to if you have any restriction on your Telephone number, or your account with current service provider has “Carrier freeze” and your account is under any of the contract or agreement with the other service provider.
IMPORTANT: If you port a number that has DSL Internet associated with it, the DSL service will likely be cut off. You should arrange for unbundled Internet service, such as Fiber, Cable, or “Dry Loop” DSL, prior to submitting your port request.
PLEASE REMOVE ANY FEATURES (i.e., Hunt Group) ASSOCIATED WITH THESE NUMBERS PRIOR TO SUBMITTING THIS LOA. ADDITIONALY, PLEASE DO NOT PLACE ANY NEW SERVICE ORDERS OR DISCONNECTS WITH YOUR CURRENT SERVICE PROVIDER ON THIS ACCOUNT, AS THIS WILL CAUSE A DELAY IN PORTING YOUR NUMBERS.
